Guest Extension Agreement

We are so excited that you have made the decision to enjoy the benefits of hair extensions! We are committed to in depth consultations for extensions to give you a realistic timeline and cost estimate to achieve goals. Due to the time consuming nature of extension appointments and our hair suppliers policies, hair extension services are non-refundable. A non-refundable deposit is required to book the installation service and if the installation appointment is missed, the guest may forfeit the deposit. If extension services are rescheduled or canceled within 24 hours of appointment time, clients may be subject to an additional fee equal to 50% of their entire service scheduled for the day. Purchased hair is non-refundable. If you purchase hair and do not use all of it for your custom installation, we will store amounts significant enough to contribute to another extension installation. All extension hair will be discarded after 6 months of storage. We do not offer refunds on purchased or discarded hair.

Extension guests must also agree to our maintenance schedule to prevent any damage to your new extensions and your natural hair. Thank you for trusting us with this service, we can’t wait for you to enjoy your new hair!

On the day of your installation appointment, please ensure that you are happy with the placement of your extensions before you leave so that we may make any adjustments necessary. If extensions are properly cared for and issues outside the norm arise (bead slips/corner comes up) we will adjust it for you at no cost (within reason, subject to stylists discretion). Issues with hair extensions related to improper care will result in additional charges.

I acknowledge the service is final after starting the application.

I acknowledge I will inspect and approve the hair to be installed by my stylist at my installation appointment.

I acknowledge that hair extensions are sensitive and different from intact human hair. I have been informed on proper care and maintenance and agree to follow daily maintenance procedures as recommended by the stylist.

I agree that in the event I no longer want to wear/have extensions I am responsible for the total payment of services rendered.

I am aware that with proper care the extension installation should last until my next maintenance appointment per the frequency shown above.

I understand if there is any matting or heavy tangling there will be an additional charge for the time needed to de-matte/detangle your extensions during removal.

I understand that my stylist is not responsible if an allergic reaction (i.e. silicone or metal allergy, contact dermatitis) occurs.

I have discussed, reviewed and agree to the pricing of my installation, maintenance and removal costs with my stylist, as shown above.

I acknowledge waiting longer than 8 weeks for maintenance is not recommended and could result in damage to my natural hair and additional costs.

I acknowledge that my stylist is not responsible for damage to my extensions or my own hair if I pursue color, extension, cutting or chemical services by another salon or stylist.

I agree to not to have any services, adjustments or removal of extensions done by any stylist not certified by the extension brand selected above.

I understand that purchased hair is non-refundable and that hair stored by Lonesome Blonde may be discarded after 6 months of storage.

I agree to book appointments per the maintenance interval above for maintenance, removal, color retouch and/or re-installation of extensions. If I cannot, for any reason, commit to the upkeep of the service, the extensions will need to be removed by a certified stylist and the hair is the property of the client. The hair may be re-installed at a later date if the extension hair was not worn for more than 6 months and as long as the stylist determines it is in proper condition for installation.

I understand that if I change the color or texture of my natural hair between the consultation and the application appointment, I agree to notify the stylist prior to the application appointment, in order to schedule a new consultation. A new deposit may be required.

I give permission for my stylist and Lonesome Blonde to use photos and videos taken before, during and after my service for marketing purposes.

I certify that the information that I supplied to my stylist during the consultation is correct. I agree to indemnify and hold harmless Lonesome Blonde, LLC free from loss, damage, claims or actions of any type that I may suffer as a result of failure to provide them with accurate and complete information.

I agree that I will not hold Lonesome Blonde, LLC liable in any way for any loss, damage or injury suffered by me as a consequence of my use/or misuse of the hair extensions.

I will indemnify and hold harmless Lonesome Blonde, LLC free from all loss, damage, claims of action or any kind that incur or that are claimed or brought against them by any party as a consequence of my own use of the hair extensions.

I agree to the terms and conditions of this contract and recognize it as a true and legally binding contract between myself and Lonesome Blonde, LLC. By checking this box I acknowledge that typing my name acts as a signature for this contract and my agreement to all boxes checked above.
